Fill it out and sign it as required then email it to: payroll@nationalsetmedics.com with your name in the Subject line.

This document must be signed by contractor requesting automatic deposit of payments and retained on file by T-Minus Productions Inc. dba National Set Medics. Contractors must attach a voided check for each of their accounts to help verify their account numbers and bank routing numbers.

If you do not have a voided check, you are responsible for assuring your numbers are correct. If incorrect, your payment will be delayed.
